HEALTH INFORMATION FORM (This is a biggie!)
The Health Information form (available in the front office) should be completed and returned to your child's teacher or to the front office as soon as possible. Having this information returned promptly is critical. The form is kept in the office in case parents need to be contacted.
Only those persons listed on this paper will be allowed to check out your child. Please make sure all telephone numbers and addresses are accurate and clearly written. In the event of an injury at school, parents will be notified. It is surprising how often we need to reach parents and emergency phone numbers are not available.
ACCIDENTS/ILLNESS
Please do not send your child to school if he/she has symptoms of illness. If your child becomes ill at school, you will be notified. Your child will rest in the health room until you are able to pick him/her up. We will not send a sick child home. The school will attempt to contact the parents first and then others who have been designated on your Health Information form.
Please inform the school if there is a change in your address or phone number. In case of serious accident or illness and we are not able to contact anyone listed on the Health Information form, the school will call paramedics. A student who contracts a contagious disease or condition, such as pink eye, chickenpox, ringworm, impetigo or head lice must remain home until the condition or disease is in control.
IMMUNIZATIONS (It only hurts for a second!)
Utah State Law requires all children enrolled in public school to complete immunizations and have a copy on file before admission to school. The office must have evidence of at least the following before your child begins school:
5 DTP/DTa/DT -- 4 doses if 4th dose was given on/after 4th birthday.
4 Polio -- 3 doses if 3rd dose was given on/after 4th birthday
2 MMR -- 3 Hepatitis B -- 2 Hepatitis A
1 Varicella (chickenpox)
MEDICATIONS
Some students may require medication during school hours. Please follow these procedures:

- Parents should make the school aware of any medical problem a child might have. School employees may not administer medicine to any student unless the school has the appropriate form signed by the child's doctor.
- An Authorization for Medication form must be filled out by the physician and signed by the parent. This form is available in the school office.
- Medications should be brought in the original container to the office by a parent with the Authorization for Medication form. Parents must initial the Receipt of Medication form in the office.
- Children should not bring any medication to school.
